The NHS Code of Confidentiality ( 2003) Protects – looks after the patients medical information Inform – ensure the patients are aware of how their information is used Provide choice: allow patients to decide whether their information can be disclosed or used in particular ways.

GP Out of Hours Service ACCESS USING THE SURGERY TELEPHONE NUMBER. YOU WILL GET TO SEE A GP LOCALLY AT ECCLESHILL (CLINIC OR A HOME VISIT IF APPROPRIATE) & THEY ARE ABLE TO ACCESS YOUR MEDICAL RECORDS IF YOU CONSENT YOU MAY BE ASKED TO REPEAT YOUR HISTORY SEVERAL TIMES THE SURGERY IS NOTIFIED ASAP
