3 3 Alabama Chapter Alabama Chapter Look how far we have come! In a recent survey, 90% of the QI team pediatricians would continue to participate in a chapter led collaborative learning project. Our pediatricians were eager to get on board! Our optimal care measures has increased from 33% to 86%! Use of Asthma Action Plans has increased from 44% to 89% Biggest Ah Ha! Moment from many of our practices: “Watching hospitalizations decrease over the life of the project and realizing we have not admitted an asthmatic patient in months”. Reports from our pediatricians on the positive experiences for patients and staff. ALABAMA

4 4 Working on 4 patient handouts with Merck for families on asthma- 3 sent out to groups to trial Sent out letters to Senior Leaders of practices at 8 months to show progress/data- helpful to engage 7 nurses/respiratory therapists from practices went to an additional 2 day Asthma Educator Course in Maine At least 4 people from practices went to an additional Motivational Interviewing Conference in Maine 91% of groups in pilot using a registry Maine Chapter Maine Chapter Last 90 Days MAINE

5 5 New Asthma Action Plans for School Age Kids Available from MaineHealth/Mai ne Asthma Council in June MAINE

6 6 Next Steps Maine is not likely going to continue in the pilot next year. Maine AAP leaders are looking at how to continue to build QI infrastructure in the state and coordinate with different groups that want to work on other topics Possible work with Improving Health Outcomes for Children (IHOC) Program that has CHIPRA money for practice improvement The Maine AAP QI Committee is looking at recommending QI measures for registry collection that include asthma MAINE

7 7 Oregon Chapter Oregon Chapter Accomplishments Establishment of future plans for ongoing asthma care improvement in already participating practices & expanding to new practices. Improved follow up care and flu shot recommendation for asthmatics. Aha! – Little Tweaks Can Cause Big Changes. OREGON

8 8 Ohio Chapter Variety, Volume, and Voracious Appetites OHIO Variety: Urban, Suburban, and Rural. Big and small. Independent and affiliated. Volume: 13 practices. Over 80 providers submitting data. Roughly 5000 encounters entered to date. Voracious Appetites: The best practice engagement ideas seem to involve food. In Memory: Gary, age 15, died of acute severe asthma, 2010.
