State Performance Plan Indicator #8 Percent of all children exiting Part C who received timely transition planning to support the childs transition to preschool and other appropriate community services by their third birthday including… an IFSP with transition steps and services notification to LEA, if child potentially eligible for Part B had a Transition conference, if child potentially eligible for Part B

Indicator #8 July 1 st, 2004 through June 30 th 2005 A. 77% of children leaving EI had an IFSP with transition steps and services B. 93% of children exiting EI had notification to LEA, if child potentially eligible for Part B C. 91% of children exiting EI had a Transition conference, if child potentially eligible for Part B.
