2 Pre Screening Tool: Vital Sign Evaluation- at medical appointments – Random CBG, Height/Weight, Age, time since last eaten on everyone age 10 and older. 4 Eligibility DPP Physical Days – Champion Provider does all DPP Physicals – We block one or two days and schedule all physicals together – At the same visit, participants get labs done and meet with DPP Staff to complete enrollment paperwork – Incentive given once they complete everything
