4 DEPARTMENT OF REGIONAL DEVELOPMENT, PRIMARY INDUSTRY, FISHERIES AND RESOURCES APPLICATION FOR AUTHORISATION An Authorisation is required to carry out mining activities on a mining interest (title). This is a legislative requirement under Section 35 of the Mining Management Act. The Department administers this Section of the Act through the Application for Authorisation. The form must be signed and the original submitted to the Department.

5 DEPARTMENT OF REGIONAL DEVELOPMENT, PRIMARY INDUSTRY, FISHERIES AND RESOURCES NOMINATION OF OPERATOR This form is required when the owner of a mining interest is not the operator for the mining site. The appointment of an operator is a legislative requirement under Section 10 of the Mining Management Act. If there is more than one title holder, each title holder should complete a Nomination of Operator form. The form must be signed and the original submitted to the Department.

6 DEPARTMENT OF REGIONAL DEVELOPMENT, PRIMARY INDUSTRY, FISHERIES AND RESOURCES AUTHORITY TO DEAL WITH AGENT This is a legal document that must be completed by both the Operator and the appointed Agent. This confirms that the consultant is the Agent of the Operator and authorises DRDPIFR to deal directly with the Agent. This Authority allows DRDPIFR to exchange and disclose information about the Operator with the Agent. All information, requests or documents will be provided directly to the Agent, and the Operator must rely on the Agent to pass on the information. Matters where DRDPIFR will deal exclusively with the Agent include all matters relating to mining activities under the Mining Management Act. Matters where DRDPIFR will deal directly with the Operator, but may also provide correspondence with the Agent, includes directions to undertake actions on a mining site.
