The use of information and records concerning individuals must be limited only to purposes directly connected with the project, including project evaluation activities. Information may not be disclosed, directly or indirectly, other than in the administration of the project unless the consent of the agency providing the information AND the individual to whom the information applies, or his or her representative, have been obtained in writing. 5

The Secretary or other Federal or State officials responsible for enforcing legal requirements have access to this information without written consent being obtained. The final product of the project may not reveal any personal identifying information without written consent of the individual or his or her representative. 6

Non-federal share (assumed by Tribe) 10% of total cost of the AIVR project To calculate the tribal 10% match, divide the requested Federal funds by 9 for each year. Example: requested Federal funds total $300,000.00 for Year 1 Tribal match = $33,333.33 7

Let the federal and non-federal budget reflect exactly what you propose in the narrative and be able to justify all requests for funding and tribal support. Are the costs adequate to serve all eligible consumers and result in gainful employment? 11

Personnel salary and fringe benefits Salary and fringe benefits for VR Counselors are reasonable in terms of the education and skill levels of each VR Counselor and the number of consumers to be served annually by each VR Counselor. 13

Travel – travel costs for project director and staff to attend annual and mid-year CANAR conferences are reasonable and customary. How will attendance benefit the project?
