Burden of Diabetes Diabetes and its associated complications costs the national economy of the US approximately 245 billion dollars annually. Diabetes is the leading cause of kidney failure, nontraumatic lower-limb amputations, and new cases of blindness among adults in the United States. The risk for stroke and heart attack is 2-4 times higher in individuals with diabetes. Diabetes accounts for 17% of all deaths for ages >25. As of today (2009) deaths related to diabetes kills more people than breast and lung cancer combined. 245 billion represents a 45% increase over 5 years. TIME TO ACT The incidence of type 2 diabetes is increasing in epidemic proportions throughout the world. Once type 2 diabetes develops, it is difficult to treat. What is the YMCA’s Diabetes Prevention Program?
An innovative, evidence-based lifestyle intervention program to help reduce the burden of chronic disease in communities across the nation. Based on the original $200 million U.S. Diabetes Prevention Program funded by the National Institutes of Health and supported by Centers for Disease Control and Prevention which showed that with lifestyle changes and modest weight reduction, the program has been shown to reduce the number of new cases of diabetes by 58%. The study proved that changing one’s lifestyle was significantly more effective than the drug Metformin in reducing the risk of developing diabetes. YMCA of the USA is working with the Centers for Disease Control and Prevention and a third party administrator and others to expand the program and bring it to as many communities as possible. 6 | YMCA’S DIABETES PREVENTION PROGRAM OVERVIEW | ©2015 YMCA of the USA

THE PROGRAM IS: A one-year program: 16 sessions over first six months, then 8 monthly sessions Led by a trained Lifestyle Coach Open to all community members; YMCA membership is not required A Centers for Disease Control and Prevention (CDC)-approved curriculum PROGRAM QUALIFICATIONS: At least 18 years old, Overweight (BMI ≥25), and Prediabetes confirmed via one of 3 blood tests or previous diagnosis of gestational diabetes If no blood test, 9+ score on risk assessment PROGRAM COST: Current/Existing Members of the Y: $150 (self-pay) Community Members: $300 (Full adult membership included for those who are not current members). Financial assistance for those who qualify and payments plans are available. For insurance coverage, must enroll via the Diabetes Prevention & Control Alliance (DPCA) at:

Individualized lifestyle program
Program Goals: To reduce the risk of developing type 2 diabetes through… ≥ 7% loss of body weight and maintenance of weight loss. ≥ 150 minutes per week of physical activity. Participants will: Learn and practice lifestyle skills related to making healthier food choices and engaging in physical activity Focus not only weight loss but weight maintenance Stress reduction and problem solving | YMCA’S DIABETES PREVENTION PROGRAM OVERVIEW | ©2015 YMCA of the USA

Trained lifestyle coaches
Create a motivating environment that is friendly and non-competitive. Facilitate group-based problem solving by utilizing motivational interviewing methods. Make learning a shared responsibility for the group rather than serving as the “expert.” Support and encourage goal setting on a weekly basis. Are accessible to participants before and after class. Qualifications Who qualifies?
Participant must have a blood value in the prediabetes range or a qualifying risk score

How you can help Help to champion program among other health care provider groups and stakeholders in the community. Provide direct program referrals to eligible patients. Add program to community-based resource directory. Post/distribute marketing materials to raise program awareness. Donate space for program sessions.
