What is Sportsware? Online database with a wide range of medical uses For Athletic Trainers: Ability to record, manage, and report what goes on in the athletic training room Medical notes, enter practice/game limitations Appointment reminders Access emergency contacts, medical information, insurance, health history, ect. Access required NCAA medical forms and physicals online

What is Sportsware? For Athletic Coaches: Look at student athletes injuries and practice/game limitations [entered by ATCs} Look up emergency contact information, insurance information, and medical information/histories Example: asthma, allergies, medications Checked “cleared status” Why they are not cleared (medical or paperwork) What paperwork they are missing

What is Sportsware? For Student Athletes Fill out and submit all of their required NCAA medical forms, physicals, insurance information, ect. online This does NOT include the NCAA forms needed by JVB, this is just the Athletic Training departments forms!! No need to print out any of these forms and have to bring a ton of paperwork with them to physicals anymore Only need to print out 2 things: Confirmation of ImPact testing Signature form

Wh at New/Transfer Student-Athletes Need to Do Step 1 : Join Sportsware

Step 2: In order for new athletes to log on, they must first be approved to join by our staff. We will be checking in on this on a consistent basis to approve students who join. Once approved, athletes will receive a conformation email where they can set their password and will be able to log on to fill out the required information. From admin@swol123.net *If they cannot find this email, make sure they check their junk mail* For returning athletes, your username and password is the same email address and password you have used to log on in the previous year(s). **If you cannot remember your sign in information, DO NOT request to join SportsWare again. Reset your password first, and if you still are having issues email one of the Athletic Trainers and they will reset your information for you.**

Step 3: When filling out information, there will be 6 different attachments [located under the “Forms” tab] that need to be filled out completely. These include the following: ADD/ADHD Form Informed Consent Form Pre-participation Physical Evaluation/Examination Form: New/Transfer Athlete Physical Form Returning Athletes Physical Form Sickle Cell Anemia Form First Agency Insurance Form Concussion Agreement Form OR

Example: Informed Consent Form Fill out form completely. **Some forms are more than two pages**

Example: Informed Consent Form When 100% done with the form, click “save and submit” and electronically sign the form. That form is now complete. If you need to come back to the form, DO NOT click “save and submit”……click “save” and you can come back to it later

First Agency Form This form must be filled out completely in order to be used If athlete is under their parent’s insurance, the carrier parent’s information must be listed This is how we submit an insurance claim if needed If athlete has their own insurance, they simply write “own insurance” under parent’s information

ADD/ADHD Form If the athlete is currently taking medication for ADD/ADHD, this form must be filled out completely. If ever drug tested, the athlete must have this on record prior to positive test in order to be accepted by NCAA “If Yes…” the student must print the document [it cannot be done online] The student fills out page #2 The student’s diagnosing doctor must fill out page #3 Return the completed form to an Athletic Trainer, or have it faxed to our office Fax # 216-373-5400 If the student is not taking medication, simply sign under “If No….”

ADD/ADHD Form

Sickle Cell Form Athletes have option to upload a copy of their sickle cell test, request to be tested, or waive the testing. If athlete is sickle cell positive, it is HIGHLY recommended to attach/bring in their records for their safety and so our staff can accommodate them best If athlete knows they are negative, they can simply waive the test [at the bottom of the form]. Most athletes in this generation are tested at birth If the athlete does not know their status, they can request to be set up to be tested Sickle cell anemia is most common in people whose families come from Africa, South or Central America (especially Panama), Caribbean islands, Mediterranean countries (such as Turkey, Greece, and Italy), India, and Saudi Arabia.

Sickle Cell Form If a returning athlete, they simply sign under where it says “Are you a returning athlete to Notre Dame College? If yes….” and then save and submit the form.

Insurance All student athletes must have insurance in order to participate in sports at NDC Front AND Back copy of insurance card must be uploaded. Entering in the information is not enough!

Step 6: ImPact Testing Every student-athlete must take the concussion ImPact test each year in order to be cleared for participation Step #6 Link is at end of step #6, and the code is located in the paragraph If having trouble, make sure pop-up blocker is off Avoid using google chrome, tends to have issues opening it and running the test Print out confirmation at end of test and bring with them to physical

Step 7: Signature Form This acts as a hard copy signature for all the information they have provided and documents they have signed (located on athletic training page). Printed out and brought with them to physicals

Uses For Coaches: Practice/game limitations and notes **Names are covered for privacy/HIPPA** **Your account will only show your athletes, this is the ATC account, so we see every sport**

Travel Report Will contain each athletes following information: Name and birth date Emergency contact(s) Insurance information Medical information Example: allergies, medications, asthma, primary doctor, ect. Medical notes Opens as a single PDF file, with names in alphabetical order for easy searching Can be printed and taken with you when traveling, or accessed from laptop, computer, Smartphone, tablet, ect.

Paperwork Report Allows you too see what paperwork each athlete is/is not missing Allows you to be able to tell the athlete(s) exactly what they still need to do to be complete with Sportsware, and therefore be “cleared” after physicals
