Fountain, CO Fountain Valley Community Services started in 1997 Committed Citizens Catholic Health Initiative had funded a Community Indicators Project In January 2005, EPCDHE joined Fountain Valley Healthy Community Initiative and introduced MAPP and PACE/EH

El Paso County Department of Health and Environment 17 Next Steps for EPCDHE Expand community ownership of process Form task forces to address top priorities EPCDHE provides support but does not take the lead Replicate the Fountain model in other communities in El Paso County

El Paso County Department of Health and Environment 18 Insights & Lessons Need a champion with the ability to execute the plan. Work with the willing. Enlist partners to accomplish goals. Document the process. Avoid paralysis by analysis. Measure and display results early, often and regularly.
