State of readiness: State of measure development and endorsement status Preventable burden: Evidence that measure could support potential improvements in population health and reduce burden of illness Supports health risk status and outcomes assessment: Supports assessment of patient health risks that can be used for risk adjusting other measures and assessing changes in outcomes, including cross- cutting measures of risk status, functional status, and condition-specific measures Enables longitudinal measurement: Enables assessment of longitudinal, condition-specific, patient-focused episodes of care to address previously unmet needs within population/public health 3 Four Guiding Principals for our Team

1.Healthy Lifestyle Behaviors—Longitudinal outcome measures of improvement (or lack of improvement) resulting from patient health-related behaviors 2.Effective Preventive Services—Longitudinal outcome measures of improvement (or lack of improvement) resulting from the use of preventive health care 3.Health Equity—Longitudinal outcome measures that evaluate the quality of health care across priority populations to track and prevent inequities and health care disparities 4 We have Identified Three Population and Public Health Sub-Domains
