Special Health Conditions Information List any medical problems your student may have. List any medication your child takes daily or as needed.

Health Exam All shots have to be up-to-date Exams needs to be done no earlier than 6 months prior to school starting. A TB test needs to be done and read in mm. The form needs to have a doctor’s stamp.

Dental exam California state law now requires a dental exam. It needs to be done no more than 12 months prior to entry. Free Dental screening from the Rotary Club June 4 th from 9am to noon at the Yale Loop Conference Center.

Medications A medication form signed by the doctor and the parent, the reason for the medication, the name, dosage, time, route and specific instructions for emergency treatment must be on file at school. Medication must be delivered to the school by the parent/guardian. Medication must be in your child’s original, labeled pharmacy container written in English. A separate form is required for each medication.

Illness guidelines for attendance The student needs to be fever-free (less than 100°F) for at least 24 hours. The student is to remain at home if they have vomited within the past 24 hours. If a student has diarrhea 2 times in a day, they need to remain at home until it subsides. Student is to remain at home with any undiagnosed skin rash until the rash has cleared, or doctor clears them.

Injured Students Parents need to contact the nurse when a child has a serious injury or surgery. Provide written information from your physician as to your child's level of activity at school.
