How to Use the Heart & Stroke Foundation’s Blood Pressure Action Plan™ Using the Mentorship Discussion Form, review with the participant their identified goals in combination with their risk profile Provide participant with relevant targeted educational resources Print out 2 copies: one for the participant and one for their on-site file File their Mentorship Discussion Form with participant’s information 9

Assist participant to measure their blood pressure Record on risk profile recording form Obtain the participant’s mentorship discussion form and their personalized Heart & Stroke Blood Pressure Action Plan™ Review actions taken since last visit using a new mentorship discussion form Provide encouragement/support 10
