Disability Income Rider - Two Options: Occupational Disability Income Occupational Disability Income – 24 hour coverage No workers compensation or state mandated plan required; occupational underwriting; some occupations are ineligible. Non-Occupational Disability Income – Available for persons in any occupation as long as the applicant has workers compensation on-job coverage or similar state mandated plan. Covers only non-occupational sickness & accidents. II

Accelerated Death Benefit Rider with Nursing Home Provision In the event insured is diagnosed as terminally ill (life expectancy of 12 months or less) or If insured enters a qualified institution with the expectation that such confinement shall be permanent, then 80% of the Death Benefit Company will pay up to 80% of the Death Benefit lump sum or, upon request, in equal monthly installments for a fixed period II Benefit Triggers:
