Access to Online Courses Request Form Draft – 4/15/08 Per the Academic Employee Master Agreement, all classroom/worksite/electronic visitations for the purposes of evaluation must be scheduled and mutually agreed upon. The most effective way for a Dean, Departmental Chair or Tenure Review Committee to observe an online course is to meet with the faculty member in person, who will open up their course on the computer and show the evaluator all areas of the course available to students. There are, however, times when a faculty member might want to give direct access to an evaluator. In these instances, the faculty member who teaches the course must give written consent to the CIDDE staff in order for the evaluator to be added into the course. Permission will normally be given to access the course in the “student” role for a period of one week, unless otherwise indicated by the faculty member. For access to be granted, the following form must be filled out by the faculty member who teaches the course and then submitted electronically to one of the DE Specialists. Faculty Member Name: _____________________________________ Course Ticket #: _____________________________ Name of Person Requesting Access: ___________________________________ Title: _____________________________________________ Access in Role of ______ Student (default) or ______ Instructor Week of Access: _________________________________ Access to Online Courses Request Form Draft – 4/15/08 Per the Academic Employee Master Agreement, all classroom/worksite/electronic visitations for the purposes of evaluation must be scheduled and mutually agreed upon.
