1 Improve & Expand Public Facilities The City will provide funding for the replacement of deteriorated sidewalks in low income residential areas designated by CDBG. 2 Expand Public Service Offerings The City will provide for (directly and indirectly) a variety of public services for residents of the community. 3 Code Enforcement The City of Waukegan will continue to provide CDBG funding to support the efforts of the Code Enforcement Division. Economic Development Opportunities The City, depending on the applications received and funding available, will continue to support efforts to improve the economic opportunities for low-income residents of the jurisdiction. 5 Infrastructure Improvements The City will provide CDBG funding for infrastructure improvements and expansion throughout the community.

City of Waukegan CDBG City of North Chicago CDBG Lake County CDBG Waukegan CDBG Funds must go to programs with beneficiaries in Waukegan; North Chicago CDBG Funds must go to programs with beneficiaries in North Chicago – the red areas. Lake County CDBG Funds must go to programs that have a significant percentage of beneficiaries in the green area – that is, within Lake County, but NOT within Waukegan or North Chicago. CDBG Public Services must be either: A new service; or A quantifiable increase in the level of an existing service New or continuing access to a service/benefit Improved access to a service/benefit A service/benefit that is no longer substandard

All applicants must provide funding for a portion of the proposed Total Program Cost from other sources. CDBG: at least 20% of the Total Program Cost If Total Program Cost is $100,000, we expect you to provide $20,000 minimum towards the program. ESG: at least 50% of the Total Program Cost Dollar for Dollar match

Eligible Applicants/programs must: be either units of government, public not- for-profit entities or private not-for-profit entities; be open to all residents of the county and must target beneficiaries outside North Chicago and Waukegan to be eligible for Lake County CDBG funding; have applications for: CDBG: a minimum of $25,000 and no more than the maximum of $50,000 ESG: a minimum of $25,000 and no more than the maximum of $50,000

Applications for North Chicago Public Service activities must target residents of North Chicago 20% Match Required If total program cost is $100,000, we expect you to provide $20,000 minimum towards the program.

Applications for Waukegan Public Service activities must be a minimum of $5,000 and target residents of Waukegan 25% Match Required If total program cost is $100,000, we expect you to provide $25,000 minimum towards the program.

ESG recipients are required to participate in the ServicePoint database ◦ UNLESS you are a domestic violence service program Collecting all data elements required in the HMIS data standard Must meet ServicePoint Participation requirements, which includes: ◦ Regular participation in monthly ServicePoint meetings ◦ Timely and accurate entry of minimum data standards Participation in the annual PIT and AHAR

CDBG-PS applicants - describe any assistance the program provides to address the action steps detailed in the Analysis of impediments. ESG applicants - discuss coordinated entry efforts and efforts at diversion including the effectiveness of your efforts.

An outcome is a problem-related, attainable, and measurable statement of a program’s intended effect on its client’s knowledge, skills, attitudes, behavior, or condition. OR The measurable effect on someone as a result of what they did in your program. A number & percentage of clients will accomplish Y for/in Z amount of time For example: 75% (10) of participants will report a 40% increase in how often they read to their children
