3.  What happened: Yacht caught fire while moored – no one on board Multiple agencies responded San Juan Fire District 3 San Juan County Sheriff IOSA Vessel Assist  Significance: Fire extinguished but vessel sunk at pier when tide shifted GDS contracted by RP (Yacht Broker) to raise vessel, remove fuel and remove from water Minimal release of pollution – all contained and cleaned in place

4.  What happened: CG responded to “mayday” call Vessel sank just west of Guemes Island Vessel was carrying fish food for salmon farm pens 3,000 gals of diesel potentially on board No injuries, casualties, only minimal pollution RP hired GDS for pollution mitigation and salvage of the vessel  Significance : Extremely challenging salvage due to the extreme currents and small “tide windows” Vessel refloated and hauled out before fuel removed due to safety concerns at location

5.  What happened: Missed turn and grounded just north of Lagoon Point Vessel was carrying cleaned salmon in chilled saltwater tanks 8,500 gals of diesel on board  Significance : No injuries, casualties, or pollution Excellent coordination with State, RP and OSRO Vessel pulled off safely at next high tide

6. WHAT HAPPENED:SIGNIFICANCE:  Vessel sank at pier while moored adjacent to M/V GOLDEN WEST  Product was removed by CG a year prior – preventing significant discharge  Ownership is disputed  Lack of “significant and substantial threat of discharge” precludes further action by FOSC other than monitoring.  CG unable to remove due to statutory constraints  Working closely with State to allow them to find creative solution for removal

7.  Due to concerns over the residual product released by the HELENA STAR, CG and Ecology conducted a Pollution assessment  Assessment revealed an estimated 10,000 gallons of product.  RP/owner contracted GDS to remove  10,500 gals of recyclable oil was recovered.  Another 1,000 gals of oily water removed
