3 INFORMED CONSENT REQUIREMENTS 1. All studies must have informed consent prospectively obtained from each research subject (or LAR) 2. Consent must contain all of the required elements of consent listed in 45 CFR 46.116 3. Informed consent must be documented using a written consent form approved by the IRB and signed by the subject or LAR. A copy must be given to the person signing the form.
