28 DRAFT AHLTA and ADM… W We’ve had some growing pains — Multiple reasons Midtier file being inappropriately placed End User documenting injury after the date of the encounter Link to transfer codes from AHLTA to CHCS is not always set correctly at the facility level Codes in AHLTA do not match codes in CHCS

29 DRAFT AHLTA Codes W Issues have been identified with ICD-9 — Incomplete codes, missing 4 th and 5 th digits — Verbiage is incorrect W Evaluation and Management Codes — Calculator works great with structured documentation

30 DRAFT How to Resolve W First Step —Gather detailed information (screen shots, reports, identifiers to locate record for research. W Second Step —Call local help desk to log Trouble Ticket Be prepared to give additional information and / or screenshots —Identify if it is a Patient Safety Issue Patient safety issues expedite the Trouble Ticket process. W Third Step —Forward patient safety issue to local users and command as informational broadcast (until it can be fixed)
