2. Welcome back to school!  This morning you will receive the following things:  Schedule  Planner  Papers that your parent/guardian need to sign and YOU need to bring back to school.  It is very important that you bring these home to your parents for them to review and sign!  Bring them in SIGNED as soon as possible and give them to your Wildcat Family teacher. All forms are due to the main office by Thursday, August 14.

3. Schedule & Map of School  On your map, circle/highlight the classrooms you will be going to each day.  You MUST follow your prescribed schedule!  Schedule changes will be made for extenuating circumstances only!  In order to request a schedule change, you must fill out the proper paperwork (see me) and return it to the main office with parent and teacher signatures.  The following reasons are not valid reasons to request a schedule change.  You dislike/don’t get along with the teacher.  You don’t want to take a certain subject/elective.

4. Parent/Caregiver Resources  Bring this paper home for your parent/guardian to read over.

5. Compact for Student Success  There are two papers in this folder.  BOTH need to be signed by a parent/guardian.

6. Application for Free & Reduced Price Lunch  If you were on Free or Reduced Lunch last year, you may eat in the same category while your new application is being processed.  This needs to be completed and signed by your parent/guardian.

7. Sexual Harassment Policy Brochure  Bring this home and read over it with your parent/guardian.

8. Update for Meal Charges  Bring this home and read over it with your parent/guardian.

9. Public Information/Communication Release Form  Write 7 th grade on the top right hand corner of this form.  This needs to be completed and signed by your parent/guardian.

10. Emergency Cards  This needs to be completed and signed by your parent/guardian.  You will not be allowed to check out if you do not turn this card in!

11. Student-Parent Compliance Forms  This needs to be completed and signed by your parent/guardian.

12. Technology Use Agreements  This needs to be completed and signed by your parent/guardian.

13. Insurance Forms  This needs to be completed and signed by your parent/guardian.  These are to be mailed in.

14. Louisiana Residency Questionnaire Form  This needs to be completed and signed by your parent/guardian.

15. District Handbook  I will pass around a form that you need to sign. By signing the form, you are stating that you received a handbook and you are responsible for knowing the information.  Turn to page 7 and find the section labeled “Student Dress Code for Males”

16. Letter from Superintendent  Bring this paper home for your parent/guardian to read over.

17. Planner  Open your planner to the first page and write your first & last name in it.  If you lose your planner, you will have to buy a new one!  Your planner is used…  to keep track of your assignments, tests, & quizzes.  as a bathroom pass for some teachers.  as a way for your teachers to communicate with your parents.  In pencil, write down your schedule somewhere in your planner.

18. Remember!  These forms are very important and need to be turned in to your Wildcat Family teacher as soon as possible.  Do not wait until the last minute to turn these forms in!  Mark the following in your planners:  ID/Yearbook pictures will be taken _______________.  Open House will be on Tuesday, September 1st.
