Cougar Care Independence & Confidentiality If you are under 18 years old, HS requires parental consent form on file; CSL cannot guarantee confidentiality If you are over 18 years old, you can give medical consent for treatment & we encourage you to discuss your medical decisions with your parents The Health Insurance Portability and Accountability Act of 1996 (HIPAA) addresses the security and privacy of health data

Cougar Care Immunizations Illinois Register Department of Public Health Title 77 Immunization Code 695 requires Tetanus Diphtheria TD within the past 10 years is required TDaP now available as a booster vaccine Measles Mumps Rubella A series of 2 is required if born after 1956 SIUE recommends all students consider Meningitis (1 shot) Hepatitis A (series of 2 shots) Hepatitis B (series of 3 shots)
