Goals of our monthly webinars Our goals for our MFP monthly webinars are: To provide training on key topics To better the lines of communication between you and us, the MFP staff To seek feedback on how to better the processes of MFP/waiver services for our participants To provide the opportunity to share successes and challenges as RTC’s and MFP transitions

MFP Transition Numbers As of today May 21, 2015 112 referrals have been received 27 participants have transitioned to date (15 in 2014 and 12 thus far in 2015) 6 more transitions planned for the 1 st part of June.

RTC’s have asked “is there a way for pre-payment for MFP Transition Services?” MFP Guidance for pre-payment of MFP Transition Services Review pre-payment framework limited use How to receive pre-payment funds Billing for MFP Transition Services after receiving pre- payment funds Participants who do not move into the community Use with caution an on a case by case basis. Work closely with MFP staff

Can MFP Transition Services be billed for Durable Medical Equipment? 2). MFP Transition Services and Durable Medical Equipment (DME) Follow your waiver policies for DME Request through state plan DME If denied, decide to cover as a waiver service (ex. specialized medical equipment) Consider MFP transition services on a case-by-case basis

Waiver Wait Lists Last month we were asked “How long should we keep MFP referrals on the wait list? Since MFP partners with 4 different waivers we have put the topic of “wait lists” on our next MFP steering committee meeting agenda for Wednesday, May 27 th. We will share the direction we are given with you on our next MFP webinar set for Thursday, June 11 th.

Successful Transition Story Linda Simmons with Lewis & Clark County Medicaid Waiver served as the RTC. Participant transitioned from a nursing home to an Accessible Spaces apartment at Aspen Village with the SDMI waiver in August of 2014.

Other Items Please make sure that you discuss your intentions and cost breakdowns for transition service expenses with Hazel prior to transition. Supporting receipts must be submitted before PA can be approved. MFP is working with Shortgrass to explore the possibility of accessing Casewave and to develop MFP demonstration services cost plans.
