1 Carver County and Scott County February 2014

2 Children’s Mental Health Case Management seeks to improve the quality of life for children with severe emotional disturbance (SED) and their families by connecting them with needed health, mental health, social, educational, vocational and recreational services. We seek to build on the existing strengths of families by partnering with them to increase the coping and solution seeking capacities of each family we serve.

3  The child must have a formal DSM IV diagnosis that limits their capacity to function in primary aspects of daily living. This DA must have been completed within the past 180 days.  Must also have at least one of the following:  Has been admitted within the past 3 years or is at risk of being admitted to inpatient treatment or residential treatment  Is currently receiving inpatient or residential treatment  Has one of the following as determined by a mental health professional  psychosis or a clinical depression  risk of harming self or others as a result of an emotional disturbance  psychopathological symptoms as a result of being a victim of physical or sexual abuse or of psychic trauma within the past year.  The impairment has lasted at least one year or that, in the written opinion of a mental health professional, presents substantial risk of lasting at least one year.

5  Bachelor’s Degree in one of the behavioral sciences or related fields from an accredited college or university  At least 2,000 hours of supervised experience in the delivery of mental health services to children with emotional disturbance  Be skilled in the following: ◦ Identifying and appraising the child’s needs ◦ Setting and monitoring appropriate service outcomes  Be knowledgeable about local community resources and how to use the resources for the benefit of the child and the child’s family

6 Functional Assessment ◦ Comprehensive & Holistic ◦ Created in collaboration with the family and providers ◦ Strengths, service needs, goal setting, specific activities, measurement Categories  Mental Health Use of drugs and alcohol  Vocation and education  Social; leisure time  Interpersonal  Self – Care/IL  Medical/Dental Health  Financial, Housing & Transportation

7 Complete an intake assessment to identify strengths and needs of child and family. Develop a service plan that outlines needs, goals and services. Help make referrals for services/supports. Help families complete MnSURE applications (for MA). Assist families in developing safety plans and assist with crisis intervention. Meet with your child/family at least once per month to monitor progress and address concerns. Provide information about community supports/activities for your child/family. Communicate with teachers, therapists, and others to assess needs and monitor progress.

8  Case management services are limited; the following services are not provided by a case manager: therapy/ counseling parent skills training family or custody mediation crisis response transportation school or legal advocacy  Case management services can help you get connected to needed services as defined through your case plan goals.

9  Children’s Mental Health Case Management is a voluntary services. Services are covered by MN Public Health Care programs – MA, Pre-paid Medical Assistance and Minnesota Care programs. There may be fees associated with case management services. Your Case Manager can assist you with your questions about how parental fees are assessed for MA. In addition, there may be fees for other services that are a part of the case plan. Ask your Case Manager regarding fees for specific programs.
