8 AFL Questionnaire Click the Advanced Settings tab

9 AFL Questionnaire Click the Questionnaire Type drop down list to chose the type of questionnaire If you plan to use the same questionnaire multiple times you might be better of choosing a Template questionnaire to use throughout your course(s).

10 AFL Questionnaire Chose Template from the dropdown list Enter the title of the questionnaire Click to Save changes

11 AFL Questionnaire Click the Questions tab

12 AFL Questionnaire Click the Add selected question type button Click the dropdown box and click the Yes/No option

13 AFL Questionnaire Enter the Question Name Click Yes to force the learner to answer the question. Enter the question text to display to the learners. Click to Save changes

14 AFL Questionnaire The question will be displayed in a list as below: Move question up Move question down Move this question Edit this question Delete this question

15 AFL Questionnaire Click the Questions tab Click the dropdown box and click the Essay Box option

16 AFL Questionnaire Enter the Question Name Click Yes to force the learner to answer the question. Enter the question text to display to the learners. Click to Save changes

21 AFL Questionnaire Preview of the completed questionnaire. When happy with how the template questionnaire looks we can create the actual questionnaires for the learners to complete.

23 AFL Questionnaire Click to hide the Template Questionnaire from learners.

32 AFL Questionnaire Amend the title of the questionnaire from the template title to the required title. Click Save Settings
