13 Sustain Plan cont’d. What steps are being taken to ensure changes will stay in place? Procedures and definitions are documented for each of the 5 changes. Barriers identified and formally addressed in documentation (e.g. family presence foremost at service team meeting; what happens if FPP worker cannot be present at Family Court… )

14 Sustain Plan cont’d. What is the target sustain measure---the point at which the Change Team would intervene to get the project back on track? Re-entry of 10% or greater

15 Sustain Plan cont’d. What system is in place to effectively monitor the sustain measure? The Change Leader, Kristie Combs, tracks and reviews monthly data on families. Rita Jeffers oversees sustain procedures at the County level. Rita and Kristie meet monthly to discuss observations.
