Extended Care = Long-Term Care

A need for care is created by two types of impairments: Physical: A chronic medical condition that compromises the individual’s ability to get through the most basic of daily routines Cognitive: A measurable decline in intellect that compromises a client’s ability to safely interact with his/her environment

Extended Care Services or Long-Term Care Services may include help with activities of daily living, home health care, respite care, hospice care, adult day care, care in a nursing home or care in an assisted living facility. It may also include care management services, which will evaluate the clients needs and coordinate and monitor the delivery of extended services. Extended Care can be all-consuming. Requiring assistance or supervision on a 24 hour basis
