Aims of the guide To provide trusts with a ‘How to’ guide To enable organisations to self-assess their position against the 4 priority standards To help organisations to implement a robust data capture method and input data into the 7DSAT To support national consistency of baseline assessment against the 4 priority standards

Background Following discussions with the Academy of Medical Royal Colleges the following four standards have been identified as having the most impact on reducing weekend mortality Standard 2: Time to Consultant Review Standard 5: Access to Diagnostics Standard 6: Access to Consultant-directed Interventions Standard 8: On-going Review Each Trust is asked to support the establishment of a robust baseline showing the extent to which these standards are being met, utilising the NHS Improving Quality Seven days Service Self-Assessment Tool (7DSAT) www.7daysat.nhs.uk www.7daysat.nhs.uk Submission to the 7DSAT needs to be completed by Friday 4th September 2015 Results of the baseline survey will be published nationally and will provide a baseline against which the progress of implementation can be measured on a quarterly basis This baseline will be used to track the progress against the roll-out of the standards nationally.

What action needs to be taken? Identify a lead individual who is responsible for collecting and inputting data into the 7DSAT Identify an executive director (Medical Director or identified other) to quality assure the data prior to submission Data can only be entered onto the 7DSAT for the purpose of baselining the 4 priority standards between 7th August and 4 th September 2015 Approve data for the trust by 5 p.m. no later than the final submission date Friday 4 th September 2015

How to gather, quality assure, and input data Ensure there is an agreed local understanding of the definitions for the seven day services clinical standards (10 Clinical Standards) (7DS FAQ).10 Clinical Standards7DS FAQ Gather information in relation to the 4 priority standards through discussion with staff in the relevant specialties Quality assure information using case note review as a method Enter a new record into the 7DSAT tool Approve the new record and ensure sign off by the identified executive director by the 4 th September 2015

Quality assuring data using a case note review To evidence the clinical standards Complete a case-note review of ten sets of case notes against each of the specialties relevant to the organisation. Case notes sample: Emergency patients admitted within the last 3 months. 5 sets of case notes from week day admissions and 5 sets of case notes from patients admitted on both days at the weekend per relevant specialty Resources for case note review: Data collection template to undertake the case note review Ten clinical standards Frequently asked question for clinical standards Data from case note review is not required to be submitted into the 7DSAT, but will inform your self-assessment
