7 Draft Options Counseling definition: Options Counseling is a person-centered, interactive, decision-support process whereby individuals are supported in their deliberations to make informed long-term support choices in the context of their own preferences, strengths, and values. The process may include developing action steps toward a goal or a LTSS plan, and, when requested, assistance in accessing support options. It also includes following-up with the individual. Options Counseling is available to all persons regardless of their income or financial assets. Workgroup developing minimum National Options Counseling Standards and evaluation guidelines Overview: Options Counseling

www.adrc-tae.org 8 Overview: Care Transitions Care Transitions refers to coordinated care, activities or services for when individuals transition from one care setting to another An aspect of person-centered hospital discharge planning Creates linkages that ensure people have the information to make informed decisions and understand their support options

www.adrc-tae.org 9 Overview: Veteran Directed Home and Community-Based Service Program (VDHCBS) Package of services that the VA purchases from AAAs on behalf of eligible veterans Provides veterans the opportunity to self-direct long-term supports and services that enable them to avoid institutionalization and continue to live independently at home AAAs assist with assessment and care planning, arrange fiscal management services, and provide ongoing options counseling and support
