2 Our Current Position 1 W.E CCG have completed the roll out of our Staff Volunteer Programme in August 2014 - We have appointed 3 Volunteer Champions within the organisation to enable implementation 2 Community Mobilising Steering Group Formed – June 2014 – embedding the CCG approach to partnership and Collaboration. Enhancing and joining up what is working well. 3 Developed and commenced roll out of co-produced, volunteer led Primary Care Based Wellness hubs. Provider Age Uk/Sept awarded 1 year pilot September 2014 for delivery in 5 initial pilot GP Practices 4 Sought and secured the buy-in of our membership and partners – delivered a partnership approach to complimenting the work of Community Agents

3 Focus: Prevention, Self Care and Evaluation in Partnership 1 W.E CCG Partnership – Collaboration, Integration and Co-production 2 WECCG/ECC integrated approach to 3 year grant funding and Market Development 3 Development of Upshot evaluation platform to deliver evidence base for Community Mobilisation 4 Development of Supporting New Ideas Pathway for April 15

Serviced by Personal Independence Coordinator AND practice staff to offer advice, signposting and information relating to the community and voluntary sector GP Based Community and Voluntary Sector “Hub” Referral Service Includes navigation of services For ALL patients within practice Personal Independence Coordinator role Engaging patients who may wish to volunteer Agreed response times with patients followed up Combined management of physical health with holistic needs Referral service for each practice Increases efficiencies within practice Empowers practice staff to consider alternatives beyond offering appointment Home safety services Community Groups Voluntary Organisations Personal Independence Planners Citizens Advice Bureau’s CVS’s Voluntary and Community Sector Referral Service Honorary contract with the practice to facilitate… Attendance at MDT meetings Access to Personal Independence Planners (voluntary role, 10 per practice), goal- setting, self-motivation and self-care tools utilising peer-support processes. OR accessing any other required services for patients discussed within MDT. Volunteering Opportunities Practice based conduit for accessing the sector Available for discussions about patients who may be recently bereaved, carer’s, those lacking in confidence/socially isolated and taking responsibility for these elements of a patients care.
