It means you and your doctor are the leaders of your medical home team assisted by: nurse practitioners, physician assistants, nurses, medical office staff and other health care professionals. In some ways, you will not notice any changes but in other ways, you will see a new focus on meeting your unique needs as a patient. So, whether you are a new patient or have been coming to name of clinic for a while…welcome to your Patient-Centered Medical Home!
