My name is Tia Olson I want to be a scientist, veterinarian, or a baby doctor. I like to know weird facts. My favorite subject in school is science. I like hands on activities. I like animals. I don’t like being told what to do if I don’t want to do it. I don’t like time limits or being rushed. INTRODUCTION

WHAT HAS WORKED WELL, WHAT HASN’T. I like to move around while I learn, I like to listen to music while I work, I like to work in groups WITH PEOPLE I KNOW. a few things that make me mad is when I get told what to do when I don’t want to do it, when I get a bad grade, when I don’t know what to do.

STRENGTHS, INTERESTS, AND PREFERENCES My strengths: My strengths are that I make friends easily, I'm kind, and I have a good sense of humor. My interests: I love music, going on the laptop, and doing anything that’s fun. preferences: I like it when teachers model/demonstration things in class. I also like to work in small groups. take multiple-choice and take open book tests.

REVIEW PAST GOALS AND PERFORMANCE Show members of your IEP team your IEP goal graphs. In 36 weeks when given a 7 minute basic math probe (decimals, fraction, problem solving,estimation), I will attempt an average of 15 problems and answer at least 10 correctly on 3/3 probes. Explain the graph to everyone. Offer explanations for why you did well, or why there may be need for improvement. Ask your IEP team for feedback.

REVIEW PAST GOALS AND PERFORMANCE Show members of your IEP team your IEP goal graphs. When given a reading probe at the 5 th grade equivalent, I will answer comprehension questions with 80% accuracy Explain the graph to everyone. Offer explanations for why you did well, or why there may be need for improvement. Ask your IEP team for feedback.

ACCOMMODATIONS My current IEP lists the following accommodations: I have a learning disability, instructor provides note, shorten assignments, extended time, time line for long assignments, frequent monitoring, extended time on test, test read aloud, calculator use, teacher provide study guide to prepare for test, language clarification. I would like the following accommodations listed on my current IEP: all the ones listed above. List accommodations that you would like to see continue on your current IEP: all of them I would like to add the following accommodations: I would like to add the accommodation word bank. I have not used the following accommodations and would like to discontinue them from my IEP: none Ask IEP team for feedback.

TRANSITION GOAL: LIVING After I graduate from high school I plan to live: I plan to live in an apartment with a friend. To meet my living goal I need to: plan where I want to live and how the bills are going to be payed and important stuff like that. I need the school and my IEP team members to help me with the following :do what your doing.

TRANSITION GOAL- EDUCATION After high school I plan to: I plan to go to college for two years. To meet my education goal: I have to work on my learning disability with math and language arts. I need the school and my IEP team members to help me with the following :Helping me with my work.

TRANSITION GOAL: EMPLOYMENT After high school, I plan to obtain employment as: I want to do something that relates to science. To meet my employment goal I need to: I want to get As and Bs in science. I want to get into college. I need the school and my IEP team members to help me with the following :I want to learn how to pay bills. I want to know about loans

GOALS FOR THIS YEAR I want to get good grades and pass my sophomore year. I also want to get to my classes early.
