8 The intent of the changes to F.S. 316.066 is to  combat Insurance fraud  place emphasis on injury and CMV crashes  capture more data with the inclusion of short forms

9  Prepared by law enforcement must be submitted to the department and may be maintained by the Law enforcement officers agency.  Must be submitted within 10 days after an investigations is completed by the law enforcement officer who investigates a motor vehicle that: ◦ Resulted in death ◦ Personal injury ◦ s.316.061(1) Hit & Run ◦ s. 316.193 DUI ◦ Rendered a Vehicle inoperable ◦ Involved a Commercial Motor Vehicle

10 FMCSA:  any truck with a GVWR/GVCWR greater than 10,000 lbs eligible to be classified as a CMV.  a motor vehicle designed to transport 9 or more people (including the driver’s seat)

11 Florida traffic statutes  Has a gross vehicle weight rating of 26,001 pounds or more;  Is designed to transport more than 15 persons, including the driver; or  Is transporting hazardous materials and is required to be placarded.  A vehicle that occasionally transports personal property to and from a closed-course motorsport facility, as defined in s. 549.09(1)(a), is not a commercial motor vehicle if the use is not for profit and corporate sponsorship is not involved.

14 These two forms are the same with the exception of the Narrative/Diagram which are not required for short form Driver Exchange form (90011S) is kept by local agency No more optional short form on Driver Exchange HSMV number is required for both short & long forms

15 1. If not a Hit and Run owner information is required. (“Same as Driver” is not acceptable) 2. A Parked Car cannot have a driver 3. If a person has a first name, they must have a last name 4. For a Non Motorist these are required fields (most, but not all, vendor applications already require this): ◦ Non Motorist Location At Time of Crash ◦ Action Prior to Crash

When “Work Zone Related” is “Yes,” these are required fields (most, but not all, vendor applications already require this):  Crash in Work Zone  Type of Work Zone  Workers in Work Zone  Law Enforcement in Work Zone ◦ (Helpful Hint: Ask your vendor if the Department Name and Department Type could be prefilled, rather than requiring the officer to do this for each crash report.)

23 11 of the highest reporting agencies are within 25 miles of each other. All agencies submitting paper reports would have to participate to make it worthwhile DHSMV would like to find an agency with extra office space to host this endeavor Encourage agencies to use a courier the reports to this central location DHSMV vendor would collect and scan reports 3 days a week. Agencies would save postage and meet timely criteria for more revenue sharing.
