5. Identifying patients with additional needs The Trust Electronic Patient Administration System (PAS) has a new flag

Ways of recording additional needs on PAS ● Emergency admission identified as having a learning disability the A&E department ● Patients admitted to ward/department who are identified as having a learning disability that is not previously recorded ● In the near future patients or their carers will be able to phone to request for the flag to be added to their record NB – the above ways of recording can ONLY be carried out with consent

7. Safeguarding All LTHT staff should have level 1 (alerter) safeguarding training LTHT has accessible safeguarding policies for adults and children which contain clear guidance for staff Lead Nurse Safeguarding Adults Child Protection Team (doctors and nurses)

8. Providing information to patients and carers New PALS leaflet New pain assessment tool in progress Plans to change the patient surveys & other patient information leaflets to make them easy read

9. Reasonable Adjustments Provide any health related information in an easy read format Offer longer appointments so that people have the time to communicate their needs and understand treatments. Offer the choice of first or last outpatient appointments
