Situation 3 The testing contractor discovered four Student Response Booklets with Post-it notes. Handwriting on Post-its didnt match the students writing. Situation 4 A teacher visually scanned Student Response Booklets at the end of the day to make sure scrap paper was removed and to see if students had answered all questions. He noticed one student had written about plans to commit suicide. The teacher photocopied the students entry and took it to the principal.
