Part D Screen Shots All the changes for Phase 2 of the Part D project are located in the MCR tab of the application processing screens. The Part D Information does not appear on the screen until either Medicare A or B are at Applied or Approved status.

Part D Screen Shots Applied/Pending Enrollment Status If Medicare A or B are at Applied Status then there are two Part D Status options, Applied/Pending Enrollment and Did not Apply/Not Enrolled. Select Applied/Pending if the applicant has contacted their Part D Provider and started the process of enrolling in Part D. (You will need the Provider and Benefit Plan name/number to enter into ASKITWeb.) Enter the Status Date. This is the date they contacted the Part D Plan. Click Find PDP Plan. This will bring up the search feature to populate the correct provider and plan.

Part D Screen Shots Applied/Pending Enrollment Status After clicking on the Find PDP Plan button, search for a provider and plan. Enter the PDP number and the benefit plan number found on the Prescription Drug Plan and Benefit Plan Reference Sheet. Access this on the KHC web site in the Medicare Part D tab.Prescription Drug Plan and Benefit Plan Reference Sheet The PDP number should always start with a letter (S, H, R, E) followed by 4 numbers (5584). The benefit plan number must be a three digit number. Enter 0 before any number less than 100. Press the search key to display the results below. Make sure the provider listed is correct and click select.

Part D Screen Shots Applied/Pending Enrollment Status Once the Provider is selected the Provider information will automatically display in the ASKITWeb Part D box. Verify that your choice displayed correctly and continue with the application.

Part D Screen Shots Did not Apply/ Not Enrolled Status If Medicare A or B are at Applied Status then there are two Part D Status options: Applied/Pending Enrollment and Did not Apply/Not Enrolled. Select Did not Apply/Not Enrolled if the applicant has not tried to enroll in a Part D Provider or Plan. Select a reason why the applicant has not enrolled. Select Didnt know I could if the client has not tried to enroll because they werent aware that they could. Select Other Creditable Coverage if the client has other coverage that they would like to use instead of Part D. (Also use this if the client has a Part D Advantage Plan or a plan number that starts with H, R, E.) Select Other for any other reason the applicant has not enrolled in a Part D Plan.

Part D Screen Shots Part A or B in Approved Status If Medicare A or B are at Approved Status then answer the QMB, SLMB, MSP question. After answering the question select one of the three Part D Status options. Select Applied/Pending Enrollment if the applicant has contacted their Part D Provider and started the process of enrolling in Part D, but has not been approved or enrolled yet. (You will need the Provider and Benefit Plan name/number to enter into ASKITWeb.) (Follow the directions from the previous slides on Applied/Pending Enrollment.) Select Did not Apply/Not Enrolled if the client has not tried to enroll because they werent aware that they could. (Follow the directions from the previous slides on Did not Apply/Not Enrolled.)

Part D Screen Shots Part A or B in Approved Status Select Approved/Enrolled if the applicant has a Part D card proving they are enrolled in a Part D Plan. Using the card, fill in the Member ID. This is the number assigned by the Part D Plan. Enter the Plan Start Date. This is the date the applicant is able to start using the Plan. Select the Find PDP Plan Button to select the enrolled provider. Follow the previous instructions on how to search for providers.
