The purpose of this Lockout/Tagout Procedure is to have a positive method of confirming that a piece of equipment, machine, or device is not energized during construction, maintenance, or renovation of equipment. This procedure shall comply with the OSHA 29 CFR 1910.147 standard (The Control of Hazardous Energy). It is the policy of the CSP to comply with this procedure to the fullest extent. 4a

Process of preventing the flow of energy ◦ Consists of installing a lock, block, or chain ◦ Requires an Authorized Employee to apply/remove lock 2a

Process of placing a warning tag on the energy source ◦ The tag acts as a warning ◦ The tag must clearly state “Do not operate” ◦ The tag can be used when the machine won’t accept lock ◦ The tag must be applied/removed by an Authorized Employee 2b

Authorized employee: A person who locks out or tags out machines before the servicing or maintenance of equipment can proceed. Affected employee: An employee who will do work on the equipment An employee who works in the area Cannot help authorized employees service machinery Can be considered an authorized employee 1b

The energy sources can take several forms: ◦ Electrical ◦ Mechanical ◦ Pneumatic ◦ Hydraulic ◦ Chemical ◦ Thermal ◦ Water under pressure ◦ Gravity ◦ Potential energy 3a

The following is a partial list of activities that require LOTO: ◦ Constructing ◦ Installing ◦ Setting up ◦ Adjusting ◦ Inspecting ◦ Modifying ◦ Repairing ◦ Cleaning equipment 5a

Employee training o Employees must receive refreshing training once a year Periodic inspections o Management will conduct inspection of a lock- out tag-out procedure in a random manner once a year o If the audit reveals that an employee is not following the lockout/tagout procedures, the employee will be required to take the refresher training 4a

Centrifugal Pumps Agitators Dry Air Compressors Wet Air Compressor Water Heater Local Control Panel Hoists Rectifier Roll up Door Ion Exchange Columns Metering Pumps Filters Any other equipment for which maintenance activities represent a risk to the maintenance operator

The following steps apply to the lockout/tagout procedure 1.Prepare for shutdown 2.Shutdown 3.Isolate equipment from energy source 4.Apply lockout or tagout device 5.Release of stored energy 6.Verify isolation 6a

Step one: Know the type and magnitude of energy source Know the hazards of the energy source Know the methods or means to control it Step two: Turn off equipment using normal controls Use orderly shutdown 6b

Step three: Isolate equipment from energy sources Secure lock to energy isolating device 8a

Step four: Notify affected employees Attach lockout/tagout devices Only authorized employee can attach or remove lockout and tag-out devices Devices must be durable Devices must identify person who applied them 9b

Step five: Relieve stored energy Verify machine isolation from energy Relieve pressure in compressed air line Add interference elements if applicable 10a Verify isolation Step six: Verify that the machine is de-energized Verify that the machine is isolated Verify that stored energy has been released

Authorized employee must have his/her own device Devices must be used to control energy only Devices could be: ◦ Locks ◦ Tags ◦ Chains ◦ Wedges ◦ Key blocks ◦ Adapter pins Self-locking fasteners 12a

Devices must have the following qualities: ◦ Be durable ◦ Be standardized in color, shape, or size ◦ Be strong enough to prevent removal Tags must have the following qualities: ◦ Be durable ◦ Be standardized in print and format ◦ Be substantial enough to prevent removal ◦ Be one-piece, environmental tolerant ◦ Include warning statements 12c

Each authorized employee must affix a personal lockout or tag-out device Each authorized employee must remove the device when work stops 14a

Lockout/tagout protection must be continuous Lockout/tagout protection must have an orderly transfer between employees 15a

The lockout or tagout device must be removed by the authorized employee who applied it The lock-out device may be removed under direction of employer if authorized employee unavailable 16a

Steps employers must take before removing a lock or tag: Verify that authorized employee who applied lock is not at the facility Take all reasonable efforts to contact the authorized employee If crucial to remove, and employee cannot be reached, call the Plant Manager or the Maintenance Operator. 16b

Inspect the work area Ensure all employees are safe Notify all affected employees 17a

A printed original of the lockout/tagout procedures at our facility can be found in the Safety and Environmental manual at the CR The locks for the machines at our facility are found at the lockout/ tagout panel by the CR The person at our facility responsible for the safety and health programs is the Plant Manager
