AU | 1300 135 600 For DEA & Member organisations DEA employees, board members, employees of member organisations & immediate family members access 24 / 7 to EAP at no cost For DEA DWW agrees to sponsorship of Annual Conference DEA to accumulate ‘Credits’ (in the form of a Bank of Hours) Formula for ‘credits’ not yet determined – based on the number of organisations, number of employees & total spend DEA to assign ‘Credits’ to member organisations to be used for On-site service delivery / Training Benefits to DEA and its Member organisations

AU | 1300 135 600 Fees (indicative only at this stage) Minimum of 500 employees required to start DEA program We are hopeful there will be a lot more $ per employee determined by total employee pool $ per organisation determined by number of employees Min. fee $990 p/a for 0-30 employees $ per employee decreases by 5% as total employee pool increases by 500 If pool 500, organisation with 100 employees annual fee = $3,500 (however, if pool 1500, then $3,159) If pool 1000, organisation with 500 employees annual fee = $16,625 (however, if pool 2500, then $15,004)

AU | 1300 135 600 How It Will Work MOU with DEA ‘Gatekeeper’ No financial commitment Service Level Agreement with each business Annual fee covers Program Management services – Program Manager, Access, Promotion, Reporting COP services – Counselling & ManagerSupport sessions On-site services are extra If project successful then ‘credits’ could well cover these costs

AU | 1300 135 600 Moving Forward – Timeline 30 November Member organisations to submit EOI to DEA and forward to DWW 31 December DWW to submit Proposal, Fee and SLA to each member organisation 31 January ( mid February) Service Level Agreements signed Commence ….? Post commencement Pro-rata so all organisations come in line for renewal at start of Year 2
