Worker Focused Safety Program Module 5 Investigations

What this module will cover O Requirements O Incident type and level O Reporting O Worker O Employer / JOH&S O Process O Results

When? O When an accident or incident: O Results in an injury requiring medical treatment, beyond First Aid; or O Results in time loss beyond the day of the accident or incident; or O Results in a near miss with the real potential for causing serious injury, or O On a deceleration of unsafe work (3.12) O Must be completed and to OH&S within 48 hours of the incident

Who Conducts? O Investigations must be investigated by people knowledgeable about the type of work involved in at the time of the incident O If reasonably available investigations must be carried out with the participation of one employer representative and one worker representative

What Is the Purpose? O The purpose of an investigation is to determine the root cause or causes of the incident, to identify any unsafe conditions, acts or procedures that contributed to the incident, and to recommend corrective action to prevent similar incidents

Who Gets Copies? O Incident investigation reports must be provided to: O Occupational Health & Safety O Administration O The JOH&S Committee (redacted) O WorkSafeBC (through OH&S)

What Happens Next? O After an investigation the employer must undertake any corrective action required to prevent the recurrence of similar incidents O The JOH&S committee should be following up on recommendations to see if they are reasonable, doable, and follow up on action taken O Follow up activities must be documented in monthly JOG&S committee meetings

The Report / Results O The investigation report should include the O Who O Where O When O What O Why O How

The Who O Employer O Injured person(s) O Other persons involved O Witnesses O Persons carrying out the investigation

The Where & When O Place (Site / Building) O Location (Section) O This should include information supporting the level of risk and injury. In some cases this may mean photos, drawings etc. O Date of incident O Time of incident

The What O Before the incident O Events leading up to incident O Process occurring before incident O What workers were doing immediately prior to the incident O Last event before incident occurred

The What O At the time of the incident O What happened at the time O What processes were occurring O What workers were doing at the time of the incident O What hazards existed O What hazards may have contributed O What personal factors may have contributed

The Why O From the answers to the “what”, identify any unsafe conditions, acts or procedures that contributed O Why did the unsafe conditions, acts or procedures occur O Why were the personal factors not identified and or addressed before the incident occurred

The How O This is the part where we get to prevent reoccurrence by: O Improving workplace inspections O Repairing or replacing faulty items O Installing safeguards O Establishing safer procedures O Training or retraining workers O Improving supervision
