4 Project FIT: Grant Timeline & Activity Review  Year 1 (2013)  CAB Capacity survey  CAB develops needs assessment survey to determine health issue Administers needs assessment survey to 10 churches and 10 community/academic organizations  Reviews findings at a church/community forum  CAB develops pilot church- based health intervention - Health education - Health Screening - Linkage to care  Year 2 (2014)  Faith/community organizations review final intervention  CAB refines the pilot church-based health intervention  Administers baseline surveys with pilot intervention and comparison churches  Implements pilot church-based health intervention to test program impact with 6 churches begins - 3 intervention and - 3 comparison churches  Year 3 (2015)  Continues to implement the pilot church-based health intervention (thru June, 2015)  CAB Capacity survey (9/3/15)  Administers 8-month survey with pilot intervention and comparison churches (beginning April, 2015)  Analyze data (beginning July, 2015)  Prepare for next grant submission

5 2013 CAB Evaluation Outcomes CAB Functioning.  96% highly agreeing: a) there is mutual respect and trust among CAB members b) CAB meetings have been a productive use of their time Internal and External Communication.  96% reported: a) their opinions were heard and discussed b) CAB meetings are clear and effective  75% reported they are providing information about the initiative within their organizations  64% reported that external communication to other organizations about the initiative's goals and accomplishments is clear and effective

6 CAB Initiative Development.  82% reported the overall amount and type of initiative strategies suggested are sufficient to have an impact on a KC African American health disparity  79% agreed that culturally and religiously-tailored strategies are being discussed and reviewed for the initiative at this time Adherence to Community-based Participatory Research (CBPR).  96% reporting the initiative is being built on strengths and resources within the African American faith community.  89% reported the initiative is well-balanced with research and action. Research Work.  79% reported research materials/procedures are culturally and religiously tailored to African American faith community values and norms  64% reported that participating churches and their members are sufficiently compensated for their involvement in the research study. 2013 CAB Evaluation Outcomes
