Registration Immunizations – state mandated (exemptions) Physical Exam - after July 1, 2014 Please come and discuss any health concerns you have for your child. Parental releases for: Allergies Asthma Heart murmurs

Medication Policy Health Office Standing Orders Physician’s Order/ Parent permission Original container Nebulizer meds- provide mask/tubing Children are not allowed to deliver medications to school

Absences All absences need to be called in to Health Office- leave a message (#2) Doctor’s note is required when your child has been absent with a contagious illness such as strep throat, conjunctivitis, etc. (See Nurse Guidelines in your packet/district website) Student should be fever free for 24 hours before returning to school.

Orthopedic Injuries If your child comes in with crutches, ace bandage, splint or arm sling please first bring them to the health office with a Doctor’s note. A Doctor’s note is needed to be excused from Physical Education Class for a period of time. No heels or open toed shoes!

Screenings Height, Weight, BMI (body mass index) Blood Pressure Vision –(Lion’s Vision Screening- Suresight vision screener) Hearing Scoliosis (age 10)

Food Policies Foods of Minimal Nutritional Value No classroom food (Birthday Board) Aramark Lunch Service Allergies can be Life-Threatening – Strategic seating and hand washing – No trading or sharing food –Epipens in stock

Getting Ready! Sleeping schedule Proper breakfast –Protein –Go, Slow and Whoa foods Review hygiene: –Proper wiping –Hand washing –Coughing/sneezing
