2 “ Together, we are transforming primary care for your better health!” Message from the Chair of the Board and Clinic Director Welcome to the Georgian Nurse Practitioner-Led Clinic (Georgian NPLC) 2012-2013 Annual Report. Since the Ministry of Health & Long Term Care’s approval of our clinic in 2009 and our full establishment in 2010 we have continued to build upon the foundation of an innovative, team- based, evidence-guided model of health care for our community. We are proud of our accomplishments in the priority areas of our clients, our people, our partners, our internal systems and our financials. Some of our most significant accomplishments include the development of our first Strategic Plan, implementation of our Electronic Medical Record (EMR), and transformation our Board of Directors towards a mature skill based composition. We have been recognized by the Ministry of Health and Long-Term Care as a role model for other nurse practitioner-led clinics for our leadership team’s pursuit of governance, operational and clinical excellence, the development of electronic performance codes, our community wide Telemedicine program and our new quality improvement plan. With 2013/2014 upon us, we look forward to working with our fellow board members, our dedicated team of administrative and health care professionals, as well as, our community partners, stakeholders and clients to meet or exceed the Ministry’s mandate and we will continue to “lead the way” for other Nurse Practitioner-Led Clinics. Feedback indicated that although they were accurate, our written statements did not succinctly convey the purpose and direction of the Georgian NPLC. With guidance from our stakeholders’ we have refreshed our core commitments. We believe that these few words now describe what we strive to do everyday. Our mission, vision and guiding values, now more clearly reflect our commitment to our clients, to the MOHLTC, and to the communities we serve. OUR CORE COMMITMENTS
