3. Cannot Use an Amendment to: Revise eligibility status (FAPE) Change disability category Location of services (LRE) Placement between public and nonpublic settings

4. Who Can Propose an Amendment? Parent School Must be a written agreement signed by both parties to amend without a meeting.

5. If Parent is Non-Responsive to or Disagrees With Request for Amendment Agreement School must hold an IEP meeting with all documentation required for an Annual IEP such as the same IEP team participation, documentation and notice procedures.

6. How to Do An Amendment in Alpine Go to the Student Measures IEP Plans Put in name of student Go to Communication Forms Click on the plus sign to add a New Sped Communication Form Record Click on Add a Form Type Select Proposal to Amend IEP Complete form.

7. What Goes Into the Form Date of proposed amendment Who the notice is to Date of the IEP that is being amended The specific item(s) you need to change. Use the location and the wording that is in the current IEP. The new wording that needs to be in the IEP. Why you are proposing the change Other options and other factors

Form continued: Name of parent who will sign the form Name of the Director Designee who allowed the amendment to be completed without a meeting How the other members of the team are going to be notified of the changes and the date they are to be notified Obtain parent signature agreeing to the amendments Scan and attach document.

10. Points to Remember Parent and school must agree to allow the amendment to occur outside an IEP meeting If parent wants a meeting to do an amendment then you do a Notice of Meeting and hold an IEP Amendment Meeting IEP Amendment Meeting is found under the SPED IEP section where you will click on IEP Amendment and the final copy will look like an IEP but be called IEP Amendment. Does not reset the Annual or Triennial dates.
