Referral Process Tel: 07816 299163 Referral agency to telephone or complete the referral form Self referral At 1 st meeting vocational profile to be completed, information given on work clubs, or signposted to another service if more suitable Attend the work club

Story so far – Since 12 th October 46 referrals 23 vocational profiles completed 20 people attending Routeone 3 people seen outside of Weston 1 job outcome Support from YES, work placement and volunteer
