4 Project Overview Purpose: Define and organize the services listed in the “Services & Resources” section of the Medical Home Portal Website: www.medicalhomeportal.org Goal: To provide clear information and easy access to valuable services and resources listed on this medical reference website for medical providers and families of children and youth with special health care needs

5 Project Overview METHODS: – Preliminary communication to solidify the process Google+ hang out, email communications, Dropbox Met with Mindy Tueller, Medical Home Portal coordinator – First protocol – solely descriptions – Evolved second/final Model protocol more comprehensive
