4 What’s important to Dianne now Company Independence Waking up gradually Knowing who’s coming to support me Making my own decisions and having choices My morning routine Going to Rendezvous disco Being treated with respect Shopping Having something sweet to eat (weight watchers biscuits or cakes) I have a morning routine in place and staff know to check with me regularly if I want to change anything I have a staff rota so I know who’s coming to support me All my staff can drive and can support me to the disco when I want to go Dianne is diabetic and has been to see the dietician with support and staff are aware of her diet and support her with this

6 What support does Dianne need to stay healthy and safe Support with personal care Staff to be aware of my routines Staff to check things out with me and listen to me and accept my decisions Having plenty of time and not to be rushed 2:1 support Support with health needs Adaptations at home I have a morning routine in place and staff know to check with me regularly if I want to change anything Dianne is diabetic and has been to see the dietician with support and staff are aware of her diet and support her with this. Dianne is supported to attend all health appointments Dianne’s home is well adapted and staff support her with any further adjustments that are needed
