*All students enrolled in 9 or more units must have comprehensive medical insurance *If an HMO, coverage within 100 miles of campus *Deductible cannot exceed $2,500 *Online waiver must be completed at pacific.edu/insuranceoffice.xml by September7 th to waive off the University sponsored plan *Must submit a copy of the health insurance card to the Insurance Office for verification by September 7 th *Waiver process must be completed each Fall or the student will not get a refund University Insurance Requirements
