2  Day Hospice  Outpatient Clinics  Cancer the Next Step  Therapies  Family Support  Spiritual Care  Grove House @ Home  CANCER-i Overview of Patient Services

3 Day Hospice  Nurse led service  Operates 4 days a week  Patients receive ongoing individual assessment and monitoring around their physical emotional and spiritual needs  Aims to assist patients in maintaining independence, managing symptoms and developing coping strategies

4  Nurse Clinic and Medical Clinic  Offers: Clinical advice Symptom monitoring and management Ongoing support and support Signposting to other appropriate services Outpatient Clinics

5  8 week programme, 3 times per year  Aim to enable patients to : Cope with diagnosis Resume as nearly as possible a normal lifestyle  For patients Aged 18-60 Recently diagnosed No secondary disease Cancer the Next Step

6  Specialist Physiotherapist Mobility or functional problems Pain, weakness, swelling or soft tissue injury Fatigue / tiredness Breathing problems  Occupational Therapy Assessment for equipment provision and adaptations Advice/support promoting independence in daily living Moving and handling  Lymphoedema Service  Complementary Therapy Reflexology, Massage, Aromatherapy, Reiki, Indian Head Massage 6 sessions for patients, 4 sessions for carers  Acupuncture  Yoga Therapies

7 Counselling Play Therapy Bereavement Support Benefits Advice For patients, carers and bereaved Family Support Service

8 Provides support whether or not someone has a religious background Struggling with unsettling questions Searching for meaning and hope Looking for inner peace and sense of well- being Re-examining their beliefs in light of their diagnosis Putting their affairs in order Spiritual Care

9  Team of 40 volunteers  Offers : Befriending Home Sitting Complementary Therapy  Visits may be weekly, fortnightly, ad hoc basis  Managed and co-ordinated by the Occupational Therapists Grove House @ Home

10  Service in Grove House drop-in, telephone helpline and email open 10am – 4pm, Monday – Friday  Offers : Friendly environment, space and time Disease-related and support information Access to other GH services  Volunteers in Hospital Clinics SACH Breast and Urology Clinics HHGH Oncology Clinic CANCER-i

11 Service Developments  Hospice at Home Nursing service – based on the Iain Rennie Hospice at Home Model  Phased introduction of the Service over the next 12 months  Team of 7 senior support / support nurses (recruitment July’08, April’09, June’09)  Macmillan Nurses will provide the Clinical Nurse Specialist (CNS) support
