4. ABC VETERINARY CENTER DISASTER PLAN EFFECTIVE: September 7th, 2008 In an effort to provide you, our client, with our mission in the event of a disaster, the following plan has been developed. Please initial each statement to acknowledge you have read and agree with all the information. _____1. Plan to take your pet with you. Public shelters will not allow pets. A list of “Pet friendly” hotels is available for you to use. _____2. If you are going to evacuate, make arrangements for your pet in a non-evacuation zone. _____3. If your pet is boarding, we will need an emergency contact person who would be able to pick-up your pet, if our facility is ordered to evacuate. _____4. If your pet is boarding and you are unable to make arrangements for your pet to be picked up, we will, as a last resort, evacuate your pet to a designated non-evacuation zone. _____5. If evacuated, your pet will be temporarily housed in an air-line approved pet carrier or crate. The crate shall be solid on all sides with a grated metal door and holes for ventilation. _____6. In accordance with the Humane Society of the United States, we have prepared a disaster supplies kit to assist us in our efforts to provide care for your pet. _____7. Using the emergency number indicated on your boarding form, and pending communication systems are functioning; you will be advised as to the welfare of your pet.

5. _____8. We make no implied or expressed guarantee that our facility or designated evacuation site could withstand a disaster. Every effort shall be made to protect your pet as well as the employees of “ABC Veterinary Center”. _____9. A “Category-3” hurricane has sustained winds of 111-130 mph and is considered a major hurricane. In the event of such a hurricane, animals will be secured and personnel will be instructed to go home and seek shelter. ____10. Mild sedation may be required for some pets; sedation will be administered at the discretion of the Veterinarian. It is in your pet’s best interest to remain calm. ____11. If our designated evacuation site is damaged, and your pet is injured, we will make every effort to provide first-aid. ____12. If your pet is severely injured, and we are unable to provide advanced medical care for whatever reason, at their discretion, our Veterinarians will humanely euthanize your pet. Caring for your pet is our utmost concern; however we do not want any pet to suffer. ____13. There will be an evacuation fee of $_______ per pet, in addition to regular boarding charges. ____14. I accept all procedures to be done to the best of the abilities of the professional staff, and I realize that no guarantee or warranty can ethically or professionally be made regarding the results or outcome. I assume all financial responsibility, and understand that payment is due upon completion of service. 7. Know your zone! Check with your county “Emergency Operations Center” Preparing for Disaster…updated every year. Know your county! What areas are prone to flooding?

8. Helpful Tips: Keep a supply of RED slip leads strategically placed throughout the facility for “In Case of Emergency” use only. Scan for microchip and photograph all incoming patients and add to “Contact Emergency Section” medical/boarding record. Notify Alarm Company that you will be evacuating the building. Plan for those patients that are in locked cages, who is assigned this responsibility? Remember…OSHA and Biohazard laws apply at all times. Add a safety notebook to supply list; include MSDS sheets for those products that you take with you. Have predetermined dilution ratios. Include all PPE in kit. Separate non-compatible substances (ACID/BASE) a distance of 20 feet, when storing and transporting. Water… when used in certain decontamination procedures can be detrimental! Refer to “2008 Emergency Response Guidebook”…very helpful with all aspects of emergency situations. Review “Volunteer Protection” and “Good Samaritan” requirements/laws as they apply to your County and State. Provide Employee’s with ID badges, for security and employee medical/exposure info.
