Action Plan Priority Area ObjectivePlanPersons Involved Resources IV) KHW Training To teach the KHWs about basic but pertinent facets of health, particularly about common ailments and diseases, as well as to equip them with basic skills necessary for them to monitor the health condition of the members of their organization 10-day seminar with volunteers where the students and available key people will lecture about selected health topics and demonstrate different basic techniques in health monitoring ASMPH Students, Key Personnel ASMPH students will provide apparatus such as stethoscopes, sphygmoman ometers, thermometer s, as well as hand-outs

Performance Measures/Targets PROGRAM / ACTIVITYPERFORMANCE MEASURES/OUTPUT TARGET 1) Formation of Health Committee Formation of an active health committee that is responsible for the health of Kalayaan members Performance of Health Committee will be measured by survey. 80% – 100% satisfaction among Kalayaan members 2) Gathering of Baseline Data and Program Evaluation Surveys and FGDs All necessary programs are evaluated

Performance Measures/Targets PROGRAM / ACTIVITYPERFORMANCE MEASURES/OUTPUTTARGET 3) Equipment Provision Funds raised sufficient enough to buy first aid kits, stethoscopes, sphygmomanometers, and thermometers for the use of KHWs. Each KHWs equipped with one stethoscope, sphygmomanometer and thermometer 4) KHW training Post-training evaluation o Written exam covering all important topics lectured during the 10-day seminar Practical Exam/Simulated patient testing wherein each Kalayaan volunteer will be evaluated in terms of accuracy of measurements and proper technique Post-training evaluation target o All KHWs will be able to get a score of 60% or more in the written exam o At least 8/10 of Kalayaan volunteers have accurate measurements and proper technique
