2 Text from ELA Project Proposal 1.The project will generate two Plans: a.Plan for developing dementia care at home b.Implementation Plan for embedding evidence- aware practice in service improvements. 2.Each Local Authority will devise its own, contextually-appropriate Plans. 3.These will be produced through joint teams, comprising LA staff, academics and other partners. 4.Impacts of Plans over 12 months: assessed 2
