Enhanced 18 Month Well Baby Visit Initiative  Child Health Program Healthy Growth and Development OPHS (2008) standard goal “to enable all children to attain and sustain optimal health and developmental potential”  Broadening of universal screening activity at the community level to include primary care practice  Relationships already established between Public Health, Community Partners and Primary Care practitioners  Leadership and supportive engagement strategy with primary care practitioners support universal developmental surveillance and promotion of early child development for toddlers and preschoolers

7. Child Health Program: Multi-pronged Promotion Strategy  PHN visits to primary care providers in Sept-Dec 2011  Provide “academic detailing” related to use of existing resources and access to web-based supports  Gather information about current practices, including provision of immunization at 18-months  Follow-up with telephone contact to determine changes in practice 3 months after the visit will take place  Parent Advocacy strategy (2012) www.18monthvisit.ca

8. Child Health Program: Multi-pronged Promotion Strategy  Approximately 75% of PCP PHN local 1 st visits complete  2012 CH Operational Plan to complete Visits/Data Collection by March 30, 2012  Begin 2 nd visits June 2012  Parent Advocacy Promotion strategy  VPD messages  eNDDS  End of 2012 – Final Data Compile/Report

Primary Care Practices want to hear from Public Health – connectedness!
